Starting Care

Insurance

​

I am an in-network provider for Aetna PPO/POS and Blue Cross Blue Shield PPO plans.

 

Prior to your first appointment, please contact your insurance company directly to inquire about the following information:

​

  • Is Dr. Fortunato in my network?

  • What are my outpatient mental health benefits for telehealth visits?

  • Do my benefits cover individual and/or couples therapy?

  • Do I have a copay, deductible, and/or coinsurance? If I have a deductible, has it been met?

  • Do I have a specific number of sessions that are covered each year?

I am considered an out-of-network provider for all other insurance plans. Should you wish to use your out-of-network benefits, I am happy to provide you with documentation that you can submit to your insurance company for possible reimbursement.

In compliance with the No Surprise Act, individuals who are uninsured or who are not using their insurance are entitled to a Good Faith Estimate explaining the cost of their care. More information can be found here.

 

First Appointment

Prior to your first appointment, you will be asked to complete initial paperwork. This is done online through a secure client portal. 

​

Payment

​

Payment is due at the time of service. I am able to accept debit/credit/HSA/FSA card payments.

​

Cancellations

 

I request 24 hours notice to change or cancel an appointment. With less than 24 hours notice, a $100 late cancellation fee will apply.

Appointments that are missed without any notice will result in a $150 no show fee.
